    Risks of cardiac tamponade resulting from uninterrupted novel oral anticoagulants and uninterrupted warfarin during perioperative period of catheter ablation of atrial fibrillation: A meta-analysis

    Objective To comprehensively assess risks of cardiac tamponade resulting from uninterrupted novel oral anticoagulants (NOACs) versus uninterrupted warfarin during the perioperative period of catheter ablation (CA) of atrial fibrillation (AF). Methods PubMed, EMBase and Cochrane Library databases were searched for studies on cardiac tamponade during the perioperative period of CA of AF with uninterrupted NOACs and warfarin from the time of NOACs application to February 2022. The Cochrane risk of bias assessment tool and MINORS (methodological index for nonrandomized studies) were adopted to evaluate the quality of literature, and Review manager 5.3 software was applied for statistical analysis on the collected data. Results A total of 13 studies involving 7, 701 patients were included. The meta-analysis showed that the difference in the risk of cardiac tamponade resulting from uninterrupted NOACs and warfarin during the perioperative period of CA of AF was statistically significant (RR=0.79, 95%CI: 0.49-1.30, P=0.36). Subgroup analysis showed that the difference in the risk of cardiac tamponade requiring thoracotomy intervention or blood transfusion between uninterrupted NOACs and warfarin during the perioperative period of CA of AF wasn’t statistically significant (RR=0.94, 95%CI: 0.48-1.86, P>0.05). The risk of cardiac tamponade wasn’t elevated in comparisons between different types of NOACs versus warfarin (P>0.05). Conclusion Uninterrupted NOACs don’t elevate the risk of cardiac tamponade during the perioperative period of CA of AF compared with uninterrupted anticoagulation of warfarin.

    Predicting the risk by FIB-4 on hepatocellular carcinoma in patients with chronic liver disease: A meta-analysis

    Objective To systematically evaluate the value of fibrosis-4 (FIB-4) in predicting future hepatocellular carcinoma (HCC) in patients with chronic liver disease (CLD).Methods We searched PubMed, Web of Science, Wan-fang and CNKI databases for relevant literatures of FIB-4 predicting HCC risk in CLD patients, and selected eligible literatures according to the including and excluding criteria. Revman 5.3 software was used to perform statistical analyses.Results A total of 35 articles with 94, 569 CLD patients were included. Meta-analysis results showed that high baseline FIB-4 predicted high HCC risk in CLD patients (HR=1.57, 95%CI: 1.41-1.75, P<0.01). Post-treatment high FIB-4 (HR=2.40, 95%CI: 1.74-3.32, P<0.01) was associated with high risk of future HCC in patients with chronic hepatitis B or hepatitis C virus. Conclusion FIB-4 is useful in predicting future HCC in CLD patients.

    Relationship between uric acid and prognosis of Chinese patients with acute ischemic stroke: A meta analysis

    Objective The meta analysis method was applied to comprehensively and systematically evaluate relations between uric acid and prognosis of Chinese patients with acute ischemic stroke.Methods PubMed, Web of Science, Embase, Cochrane Library, CNKI, VIP and Wanfang databases were searched by computer system to collect case-control studies on relation between the level of uric acid and prognosis of patients with acute ischemic stroke in Chinese from database establishment to February 2022. The Meta analysis was performed using STATA 16 software. Results Thirteen literatures representing a total of 11, 046 study subjects (n=8, 016 in the good prognosis group, and n=3 030 in the poor prognosis group) were included in the meta-analysis. The results showed that the uric acid level of patients in the good prognosis group was higher than that of poor prognosis group [WMD=18.44 μmol/L, 95%CI(3.20, 33.68), P<0.05]. The results of sensitivity analysis and publication bias test showed that the study results were stable and reliable.Conclusion The level of uric acid in Chinese may be related to the prognosis of patients with acute ischemic stroke, and the higher level of uric acid in Chinese patients may have a better prognosis.

    Predictor analysis of abnormal coronary flow reserve in women with non-obstructive coronary artery disease

    Objective To explore the predictors and clinical features of abnormal coronary flow reserve (CFR) in women with non-obstructive coronary artery disease (NOCAD). Methods We performed a retrospective analysis of 223 NOCAD female patients from the aspects of general clinical features, vital signs, left-ventricular ejection fraction (LVEF) on the echocardiography during hospitalization, nuclear myocardial perfusion imaging parameters and laboratory blood tests indexes for further statistical analysis. The patients were divided into two groups: normal CFR group (CFR≥2.5) and abnormal CFR group (CFR<2.5). Results The levels of total cholesterol and low density lipoprotein cholesterol (LDL-C) were significantly higher in abnormal CFR group than those in normal CFR group (P=0.011, P=0.002). Multiple logistic regression analysis revealed that LDL-C level was only found to be independently associated with abnormal CFR (OR=3.346; 95%CI=1.254-8.930; P=0.016). The receiver operating characteristic (ROC) curve analysis showed that the optimal cut-off value of LDL-C level for predicting abnormal CFR was 3.36 mmol/L with a sensitivity of 40% and specificity of 77% (AUC=0.609; 95%CI=0.535-0.683).Conclusion The LDL-C level is found to be independently associated with abnormal CFR in NOCAD female patients, and coronary microvascular dysfunction is more likely to occur in women with abnormal LDL-C level.

    Objective To study the efficacy of cardiopulmonary resuscitation (CPR) training mode from tertiary hospital to community health service center then to community residents via health education platform. Methods The CPR skills of the medical staff of our center were regularly guided and assessed by the experienced personnel of the tertiary hospitals, and then, CPR training were carried out in 500 community residents who participated into the health education from September 2020 to September 2021. The manner of PPT explanation, manipulator operation, health education lecture, WeChat, micro-lecture, as well as multiple training modalities in combination were conducted, two questionnaires and examination questions of pre-post-training were performed to evaluate the training effectiveness.Results The community residents significantly enhanced the willingness and knowledge to CPR training of post-training as compared to those of pre-training (all P<0.05). Conclusion The CPR training mode from tertiary hospital to community health service center then to community residents has a significant effect, further generalization for improvement of the rescue success rate of out-of-hospital cardiac arrest.

    Diagnostic value of galactomannan on the comorbidity of bronchiectasis and pulmonary aspergillosis

    Objective To compare the efficacy of galactomannan (GM) in serum and bronchoalveolar lavage fluid (BALF) in the diagnose of the comorbidity of bronchiectasis and pulmonary aspergillosis (PA). Methods Totally 128 bronchiectasis patient from the Department of Respiratory and Critical Care Medicine, Shengli Oilfield Central Hospital (March 2018- March 2021) were enrolled into observation group (n=41, including 12 diagnosed cases and 29 clinically diagnosed cases) and control group (n=82) as the diagnostic result. The serum GM and BALF-GM were respectively conducted to detect in two groups before instituting anti-Aspergillus therapy, the receiver operating characteristic (ROC) curves were drawn to compare the diagnostic value of the two detection on the comorbidity of bronchiectasis with PA and assess the cut-off values of serum GM and BALF-GM. Results The Optimal cut-off values of were 0.65 μg/L for serum GM (area under the ROC curve [AUROC] of 0.856; sensitivity of 65.4%; specificity of 88.5%) and 0.93 μg/L for BALF-GM (AUROC of 0.929; sensitivity of 80.4%; specificity of 95.0%). At a cut-off value of 1.5μg/L, the specificity and sensitivity in serum-GM were 11.5% and 100%, respectively; but at a cut-off value of 2.0 μg/L, those in BALF-GM were 19.2% and 100%, respectively. Conclusion BALF-GM is superior to serum GM for the sensitivity and specificity in the diagnose of the the comorbidity of bronchiectasis and PA, preferring to assist the diagnosis as a tool.

    Objective To investigate the characteristics of lipid accumulation product (LAP) and visceral fat index (VAI) in first-degree relatives of patients with type 2 diabetes mellitus (T2DM) who were impaired glucose regulation (IGR).Methods Totally 212 patients were as subjects among 298 first-degree relatives of T2DM. The clinical data were assessed in the three groups which comprised 115 (normal glucose tolerance, NGT), 93 (impaired glucose regulation, IGR), and 90 (T2DM) patients. Results There was no significant gender difference among the three groups (P>0.05), but differences in age, height, weight, body mass index, and waist circumference among the three groups were all statistically significant (P<0.05). The NGT group was the youngest, and the T2DM group had the largest weight and waist circumference (all P<0.05). The incidence of central obesity in NGT group, IGR group, and T2DM group were 52.17%, 67.74%, and 78.88%, respectively, and the obesity rate in T2DM group was the highest, and the central obesity rate in T2DM group and IGR group was significantly higher than that in NGT group (all P<0.05). There were statistically significant differences in LAP and VAI among the three groups (P<0.05). The T2DM group had the highest LAP and the NGT group had the highest VAI (all P<0.05). LAP and VAI were independent influencing factors for glycated hemoglobin in first-degree relatives of T2DM patients, and LAP was an independent influencing factor for 2-hour blood glucose.Conclusion The central obesity rate in first-degree relatives of T2DM and prediabetes is significantly increased, and the new indexes of LAP and VAI are important influencing factors for glucose metabolic disorders of these patients.

    Objective To determine the predictive value of neutrophil to lymphocyte ratio (NLR) for microalbuminuria of type 2 diabetes mellitus (T2DM). Methods A retrospective analysis was performed on 400 T2DM inpatients admitted to the Department of Endocrinology, Hebei General Hospital from November 2020 to June 2021. Using the results of 24 hour urinary microalbumin, the patients were assigned to the normal group (n=236, 24 hour urinary microalbumin<30 mg) and the microalbuminuria group (n=164, 30 mg≤24 hour urinary microalbumin<300 mg). The general data and laboratory results were compared between two groups, binary Logistic regression analysis was conducted to assess the influencing factors for microalbuminuria in T2DM patients. The predictive value of NLR and combined diagnosis on microalbuminuria in T2DM patients was evaluated by drawing receiver operating characteristic (ROC) curve. Results The percentage of patients with advanced age and hypertension, white blood cell (WBC) count, neutrophils absolute (NEU) count, NLR, triglyceride (TG), serum creatinine (Scr) and urea nitrogen (BUN) were higher in microalbuminuria group than the normal group, but the estimated glomerular filtration rate (eGFR) and high density lipoprotein cholesterol (HDL-C) were significantly lower than that in the normal group (all P<0.05). The area under the curve (AUC) of NLR alone for predicting microalbuminuria of T2DM patients was 0.644[95%CI(0.586, 0.701)], the best cutoff value for 2.81, the sensitivity for 46.1%, and the specificity for 85.8%. The AUC obtained by NLR combined with BUN, eGFR, TG, and whether hypertension was 0.727[95%CI (0.678, 0.777)], the optimal cutoff of 0.426, the sensitivity of 59.9%, and the specificity of 74.2%.Conclusion NLR can be used as one of the clinical indicators to predict microalbuminuria of T2DM patients.

    Objective To investigate the relationship between cognitive dysfunction and human β Amyloid 1-42 (Aβ1-42) in patients with diabetic kidney disease (DKD). Methods In this study, patients(n=161) with type 2 diabetes mellitus (T2DM) who were treated in the Department of Endocrinology, Affiliated Hospital of Xuzhou Medical University from May 2020 to May 2021 were as subjects. These patients were enrolled into normoalbuminuria (DM) group (n=60), microalbuminuria (DKD1) group (n=58) and macroalbuminuria (DKD2) group (n=43) according to urine albumin creatine ratio(UACR)levels. The patients' cognitive function was assessed by the Montreal Cognitive Assessment (MoCA) scale. Aβ1-42, UACR, Cystatin C (CysC), estimated glomerular filtration rate (eGFR) were detected, and the relationship between the above indicators and MoCA score was analyzed.Results Compared with DM group, CysC and Aβ1-42 in DKD1 and DKD2 groups increased (P<0.05), and eGFR decreased (P<0.05). Compared with DKD1 group, CysC increased and eGFR drcreased in DKD2 group(P<0.05). The MoCA scores in the DKD groups were superior to those in DM group (P<0.05), which in the DKD2 group were prevalent (P<0.05). MoCA scores in the three groups were negatively correlated with Aβ1-42, UACR, and CysC (all P<0.01), and positively correlated with eGFR (P<0.01). Aβ1-42 in the three groups was positively correlated with UACR and CysC (all P<0.01), and negatively correlated with eGFR (P<0.01). The results of multivariate linear regression analysis showed that Aβ1-42 and UACR were the influencing factors for MoCA score in DKD patients. Conclusion Cognitive dysfunction exists in DKD patients, which is closely related to the level of Aβ1-42 and the severity of renal damage.

    Effect of anti-vascular endothelial growth factor on outer retinal structure restoration of diabetic macular edema

    Objective To investigate the effect of intravitreal ranibizumab (IVR) injection on the restoration of outer retinal structural integrity in diabetic macular edema (DME). Methods In this retrospective study, totally 24 DME patients (28 eyes) were treated with intravitreal injections of 0.05ml ranibizumab with average time of follow-up was 8.21±1.56 months. Best corrected vision acuity (BCVA) of pre- and post-treatment was recorded in detail, which calibrated a logarithm of the minimal angle of resolution (logMAR) visual acuity chart in statistical analysis. Spectral domain optical coherence tomography (SD-OCT) was performed pre- and post-treatment. The characteristics of macular structural were statistically analyzed, including central macular thickness (CMT), the continuity of external limiting membrane (ELM) and ellipsoid zone (EZ).Results Mean logMAR VA of 28 eyes decreased after IVR therapy from 1.38±0.21 at baseline to 0.39±0.08 at 6 months after the first injection (P<0.05). Similarly, CMT reduced from (416.36±23.10)μm at baseline to (231.15±38.26) μm at 6 months after the first injection (P<0.05). Global ELM disruption reduced from 78.6% pre-treatment to 10.7% at 6 months after the first injection; likewise global EZ disruption reduced from 71.4% to 14.3%.There was a strong correlation between BCVA and CMT (r=0.356, P<0.01), the statuses of ELM (r=0.589, P<0.01) and EZ (r=0.626, P<0.01).Conclusion Anti-vascular endothelial growth factor (VEGF) therapy can restore the integrity of outer retinal structure (ELM, EZ) in DME patients, thus improving vision. ELM restoration preceds EZ restoration.

    Objective To explore the effect of dual coping intervention on coping ability and marital adjustment of young adult patients with breast cancer and their spouses. Methods The young ault postoperative patients with breast cancer and their spouses (n=80 pairs), who visited the Department of Breast, the International Peace Maternity & Child Health Hospital of China welfare institute (June 2021 to September 2021) were randomized to recevie conventional care (control group, n=40 pairs) or dual coping intervention + conventional care (observation group, n=40 pairs) for 4 months, aiming to compare intergroup coping ability and marital adjustment.Results Before intervention, the two groups had insignificant differences in the total scores and dimension scores of the dyadic coping inventory (DCI) scale and Locke-Wollance marital adjustment scale (LWMAS) (all P>0.05); after intervention, the observation group had significantly higher total scores and dimension scores of DCI scale and LWMAS than the control group (all P<0.05).Conclusion Dual coping intervention can improve the coping ability and marital adjustment of young adult patients with breast cancer and their spouses, which contributes to the improvements in marital quality and happiness.

    Objective To explore clinical characteristics of patients with pseudohypoparathyroidism, improve the understandings on the disease, and lower the clinical misdiagnosis rate. Methods Clinical data, diagnosis and treatment of a case of pseudohypoparathyroidism type Ia complicated with congenital hypothyroidism were analyzed retrospectively, and related literatures were reviewed.Results The 9-year-old boy was diagnosed with congenital hypothyroidism since childhood, and was misdiagnosed as epilepsy for years due to repeated convulsions. The laboratory examinations showed the patient had low blood calcium, high blood phosphorus and high parathyroid hormone, who was diagnosed to suffer from pseudohypothyroidism type Ia with congenital hypothyroidism by combining clinical characteristics and gene sequencing results. The patient was treated with calcium supplementation, active vitamin D and levothyroxine sodium and had relieved symptoms.Conclusion Pseudohypoparathyroidism is a rare genetic disease, and it is clinically manifested to be complicated and high misdiagnosis rate and missed diagnosis rate.Children with unexplained epilepsy blood calcium, phosphorus and parathyroid hormone detections at earliest convenience to detect and treat early.

    Objective To explore the clinical effect of the multidisciplinary combination in treatment of exertional rhabdomyolysis (ER) complicated with acute renal insufficiency (ARI) and catheter-related thrombosis (CRT). Methods A retrospective analysis was conduted in one ER patient complicated by ARI and CRT who visited Department of Emergency and Department of Vascular Surgery, the Third Hospital of Hebei Medical University. The clinical data and treatment plan were analyzed to evaluate the clinical treatment effect.Results Hemodialysis was used for ARI in the Department of Emergency, thrombectomy was performed to treat CRT in the Department of Vascular Surgery. The patient recovered and was discharged from the hospital.Conclusion This patient has achieved good clinical outcome through multidisciplinary combination therapy.
