社区糖代谢异常人群颈围与心血管危险因素的相关性

Relationship between neck circumference and cardiovascular risk factors in population with hyperglycemia in community

摘要: 目的  探讨糖代谢异常人群颈围与心血管危险因素的关系。方法  选取2014年7-12月大连市社区40岁以上糖代谢异常居民2 319例为研究对象。回顾性分析一般临床资料、血糖、血压、血脂等。以性别分组,校正混杂因素后通过多元线性回归和Logistic回归分析颈围与心血管危险因素的关系。结果  (1)男性676例,平均颈围(39.0±2.7) cm,女性1 643例,平均颈围(35.0±2.6)  cm,男性颈围大于女性(P<0.01)。(2)Logistic回归分析显示,男性颈围增大仍会增加糖尿病的发生风险;女性颈围增大会增加糖尿病、高血压、代谢综合征(MS)的发生风险。(3)男性和女性颈围对MS诊断的受试者工作特征 (ROC) 曲线下面积分别为0.755和0.731,最佳切点值分别为37.9  cm和33.9  cm。结论  糖代谢异常人群颈围与心血管危险因素独立相关,男性颈围大于37.9  cm,女性颈围大于33.9  cm可预测MS的发生。

Abstract: Objective  To explore the relationship between neck circumference (NC)  and cardiovascular risk factors in population with hyperglycemia in community. Methods  A total of 2319 subjects (aged ≥40 years) with hyperglycemia was recruited in Dalian community from July 2012 to December 2014. NC, waist circumference (WC), blood pressure, plasma glucose, blood lipids were measured and documented in the questionnaire along with other basic information. Linear regression model and logistic regression model were constructed to assess the association between NC and cardiovascular risk factors.Results  (1) Average NC was (39.0±2.7) cm in men and (35.0±2.6) cm in women, which was  higher in males than females. (2) Logistic regression showed that after adjusting for BMI and WC, Increased NC is associated  with diabetes in men  and diabetes, hypertension, MS  in women. (3) The AUC was 0.755 for men and  0.731  for women. The neck circumference of 37.9  cm for men and 33.9  cm for women was the best cutoff point for determining subjects with MS.Conclusion  Neck circumference is associated with cardiovascular risk factors in community population with hyperglycemia.
