摘要: 目的  探讨二甲双胍联合艾塞那肽对新发初诊2型糖尿病(T2DM)患者胰岛功能的影响。方法 将120例新发初诊T2DM患者随机分成4组:饮食+运动治疗组30例(A组),二甲双胍治疗组30例(B组),艾塞那肽治疗组30例(C组),二甲双胍+艾塞那肽治疗组30例(D组)。检测血糖、胰岛素、胰高血糖素(GC)、胰高血糖素样肽1(GLP1)等指标,计算胰岛素分泌指数(HOMAβ)、胰岛素抵抗指数(HOMAIR)。结果 治疗后B、C、D组HOMAIR下降、HOMAβ升高(P<0.05)。治疗后:与A组比较,C、D组HOMAIR下降,HOMAβ升高(P<0.05);与B组比较,C、D组HOMAIR下降,HOMAβ升高(P<0.05);与C组比较,D组HOMAIR下降,HOMAβ升高(P<0.05)。结论 二甲双胍联合艾塞那肽可改善新发初诊T2DM患者的胰岛功能,且作用效果优于两药物单用。

Abstract: Objective  To explore the impact of  exenatide  combined with metformin on islet function in patients with newly diagnozed type 2 diabetes mellitus. Methods  A total of 120 patients with newly diagnozed T2DM were randomly divided into four groups: diet and exercise group 30 cases(group A),  metformin group  30  cases (group B),exenatide group 30 cases(group C)  and metformin and exenatide group 30  cases (group D). Blood glucose, insulin, glucagon (GC) and glucagonlike peptide1  (GLP1) were measured. Insulin secretion index(HOMAβ) and insulin resistance index(HOMAIR)  were calculated. Results  HOMAIR after treatment was significantly reduced in group B, C and D (P<0.05), HOMAβ  was significantly increased in group B, C and D (P<0.05). After treatment, group B showed no significant difference compared with group A; the HOMAIR in group C and D after treatment was significantly decreased (P<0.05), and the HOMAβ  was significantly increased (P<0.05); Compared with group B, the HOMAIR in group C and D after treatment was significantly decreased (P<0.05), and the HOMAβ  was significantly increased (P<0.05); Compared with group C, the HOMAIR in group D after treatment was significantly decreased (P<0.05), and the HOMAβ  was significantly increased (P<0.05). Conclusion   Exenatide combined with metformin could effectively improve the islet function in newly diagnozed T2DM, and the effect was better than monotherapy of exenatide or metformin.
