2型糖尿病患者糖尿病周围神经病变相关因素分析

摘要: 目的  探讨2型糖尿病患者糖尿病周围神经病变(DPN)相关因素。方法  选择2型糖尿病患者114例,根据肌电图中运动及感觉神经传导速度,评估糖尿病周神经病变情况,分为DPN组73例和非糖尿病周围神经病变(NDPN)组41例,分别就其性别、年龄、病程、体重指数、腰围、空腹血糖、餐后血糖、糖化血红蛋白(HbA1c)、血脂、运动及感觉神经传导速度、颈动脉内中膜厚度、足部踝肱指数、感觉阈值等指标进行分析。 结果  DPN 组年龄、病程、空腹及餐后血糖、HbA1c均高于NDPN组(P<0.05),运动及感觉神经传导速度低于NDPN组(P<0.01),颈动脉内中膜厚度、足部感觉振动阈值高于NDPN组(P<0.05)。结果表明,高龄、病程长、高HbA1c水平是2型糖尿病患者发生DPN的独立危险因素(P<0.05)。 结论  2型糖尿病患者DPN的发生与年龄、病程、HbA1c水平有关。

Abstract: Objective  To investigate the related factors of diabetic peripheral neuropathy in patients with type 2 diabetes mellitus(T2DM). Methods  One hundred and fourteen patients with T2DM were included. Diabetic peripheral neuropathy was evaluated according to the motor and sensory nerve conduction velocity, and the patient  were divided into diabetic peripheral neuropathy group (DPN group, 73 cases) and non diabetic peripheral neuropathy group (NDPN group, 41 cases). The gender, age, duration, body mass index, waist circumference, fasting blood glucose, postprandial blood glucose, glycosylated hemoglobin (HbA1c), the blood lipid, motor and sensory nerve conduction velocity, carotid intimamedia thickness, foot and ankle brachial index, sensory threshold index were analyzed. Results  The age, disease  course, fasting and postprandial blood glucose and HbA1c in DPN group were significantly higher than those in NDPN group (P<0.05). The motor and sensory nerve conduction velocity was significantly lower than that in group NDPN (P<0.01), the carotid intimamedia thickness and foot sensation threshold were significantly higher than those in group NDPN (P<0.05).  The data showed that advanced age, long duration and high HbA1c levels in patients with T2DM were independent risk factors of DPN (P<0.05). Conclusion  DPN is associated with age, duration and HbA1c level in patients with T2DM.
