血清炎性因子及凝血功能与急性脑梗死患者神经功能预后的相关性

摘要:

目的 探究血清炎性因子及凝血功能与急性脑梗死患者神经功能预后的相关性。方法 选取2021年1月至2022年1月在南昌市第三医院接受治疗的急性脑梗死患者(观察组)80例,同时选取同时期健康体检者(对照组)40例。收集两组临床资料。结果 与对照组比较,观察组白细胞介素-6(interleukin 6,IL-6)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、超敏C-反应蛋白(hypersensitive C-reactive protein,hs-CRP)、纤维蛋白原(fibrinogen,Fib)、D-二聚体(D dimer,DD)水平均较高,血浆凝血酶时间(plasma thrombin time,PT)、部分凝血活酶时间(activated partial thromboplastin time,APTT)水平均较低(P<0.05)。观察组中轻度、中度、重度亚组IL-6、TNF-α、hs-CRP、DD、Fib水平均依次升高(P<0.05)。相关性分析显示,观察组血清IL-6、TNF-α、hs-CRP、Fib和DD水平均与神经功能缺损程度呈正相关(r分别为0.402,0.420,0.498,0.305,0.473,P<0.01),PT、APTT与神经功能缺损程度无相关性(P>0.05)。结论 血清炎性因子(IL-6、TNF-α、hs-CRP)及凝血功能(Fib、DD)与急性脑梗死患者神经功能预后具有一定的相关性。

Abstract:

Objective To explore correlations between serum inflammatory factors, coagulation function and prognosis of neurological function in patients with acute cerebral infaretion (ACI). Methods From January 2021 to January 2022, eighty ACI patients (observation group) and 40 health examiners (control group) in the Third Hospital of Nanchang were randomly enrolled. Clinical data of two groups were assessed. Results Compared to the control group, interleukin 6 (IL-6), tumor necrosis factor-α (TNF-α), hypersensitive C-reactive protein (hs-CRP), fibrinogen (Fib) and D dimer (DD) in the observation group were significantly higher, and the level of plasma thrombin time (PT) and activated partial thromboplastin time (APTT) were significantly lower (P<0.05). IL-6, TNF-α, hs-CRP, DD and Fib escalate increased consistently with ACI in mild, moderate and severe subgroups of the observation group (P<0.05). The correlation analysis showed that the level of all indexes including serum IL-6, TNF-α, hs-CRP, Fib and DD in the observation group were positively correlated with the degree of neurological impairment (r denoted 0.402, 0.420, 0.498, 0.305 and 0.473, respectively, P<0.01), neither PT nor APTT was correlated with the degree of neurological impairment (P>0.05). Conclusion Serum inflammatory factors (IL-6, TNF-α, hs-CRP) and coagulation functions (Fib, DD) are certainly related to prognosis of neurological function in ACI patients.
