伴有抑郁症状肝癌患者脑功能连接研究

摘要:

目的 结合静息态功能磁共振成像(Resting-state Functional Magnetic Resonance Imaging, rs-fMRI)探讨抑郁症状对肝癌患者静息态功能连接(Functional Connectivity, FC)的影响。方法 使用1.5T MR扫描仪对21例肝癌伴抑郁症状、27例肝癌非抑郁患者进行扫描,并获得静息状态下的功能磁共振成像(rs-fMRI)。比较肝癌抑郁组和非抑郁组之间FC的差异。之后,进一步分析功能影像指标与汉密尔顿抑郁量表(Hamilton Depression Scale, HAMD)评分的相关性。结果 肝癌伴抑郁患者默认模式网络(楔前叶、海马及海马旁回、顶下小叶)、执行网络(扣带回、额内侧回、额上/中/下回)、感觉运动网络(小脑)、突显网络(左侧岛叶)及丘脑节点脑区间存在功能连接减弱表现。而肝癌伴抑郁患者默认模式网络(顶下小叶)与感觉运动网络(左侧小脑)功能连接增强。结论 肝癌合并抑郁患者在多个抑郁相关脑区存在功能连接异常,可能与其神经病理学机制有关。

Abstract:

Objective To explore the effect of resting-state functional magnetic resonance imaging (rs-fMRI) on functional connectivity(FC) in liver cancer(LC) patients with depression. Methods Twenty-one LC patients with depression (depression group) and 27 LC patients without depression (non depression group) were treated with 1.5T MR scanner, and rs-fMRI was obtained. The difference of FC was compared between groups, then, the correlation between functional imaging index and Hamilton Depression Scale (HAMD) score was analyzed.Resultss The patients with liver cancer and depression showed significantly weakened FC in the default mode network (the precuneus, the hippocampus and the parahippocampal gyrus, the inferior parietal lobule), the executive network (the cingulate gyrus, the medial frontal gyrus, the superior / middle / inferior frontal gyrus), the sensorimotor network (cerebellum), the salience network (the left insular lobe) and the thalamic node brain interval. The functional connection between the default mode network (inferior parietal lobule) and the sensorimotor network (the left cerebellum) was significantly enhanced in patients with liver cancer and depression.Conclusion LC patients with depression showed the functional connectivity abnormalities in multiple depression-related brain regions, which may be related to the neuropathological mechanism.
